Purpose: Conflicting results have been reported between the use of conventional protocol MRI and advanced protocol MRI to avoid unnecessary histopathology when magnetic resonance imaging (MRI) is used for the diagnosis of hepatocellular carcinoma (HCC). Therefore, we aimed to compare the diagnostic performance of conventional MRI and advanced technique MRI to avoid unnecessary histopathology.

Methods: Original studies reporting the diagnostic performance of MRI for the diagnosis of HCC published between January 2010 and February 2021 were identified were performed independently by two radiologists to identify articles published prior to June 2021.

Result: A total of 3,757 HCCs and 3,682 benign liver lesions from 35 studies were included. The overall sensitivity and specificity of the diagnostic performance of conventional MRI was 0.81 (0.77-0.94) 0.78 (0.84-0.92) and Advance MRI 0.93 (0.83-0.89), 0.86 (0.79-0.96) respectively

Conclusion: The present meta?analysis suggests that Advance MRI may increase the sensitivity, and specificity for the diagnosis of HCCs
